Short Survey Questions

These are the questions in the short survey.

Question 1: Before we begin, we have to check whether you have done the longer listening survey. You can choose which survey you do, but please only do one of them. Have you completed the longer survey?

Option B: 15-18 years old
Option C: 19 -21 years old
Option D: 22-24 years old
Option E: 25-29 years old
Option F: 30 years old or more
Option G: I would prefer not to say

Question 3: Are you a person with a disability, or a carer?

Option A: I am a carer
Option B: I am a person with a disability
Option C: I am a carer and person with a disability
Option D: I am neither
Option E: I would prefer not to say

Question 4: What is your gender?

Option A: Woman
Option B: Man
Option C: Non-binary
Option D: Other
Option E: I would prefer not to answer

Question 5: The next question will ask: ‘What does good listening mean to you?’. Do you want to answer this question by uploading your response (e.g. a drawing) or by providing a written or verbal response?

Option A: I would like to give a written or verbal response
Option B: I would like to upload my response

If you chose Option A: Question 5A: What does good listening mean to you? You can record your answer or write your response.

If you chose Option B: Question 5B: What does good listening mean to you? You can answer this question however you like. For example, you can upload a photo, or a drawing.

Question 6: You are nearly at the end of the survey. You are now invited to participate in a follow up interview. In the interview, we will ask you more questions about your experiences of listening and being heard. You do not have to do the interview, but if you are interested in it, please share your email or phone number and we will contact you with more information. Giving us your contact information does not mean you have to participate in the follow up interview. You can decide after we give you more information about it. If you do not want to participate in an interview, you can skip this question.
